MICRONEEDLING – PART 1
One of my favorite subjects -MicroNeedling!
Medical microneedling of skin also referred to as Collagen Induction Therapy (CIT), is a relatively new concept aimed at stimulating the body’s own collagen and elastin production to tighten, lift and rejuvenate the skin, as well as reduce the appearance of fine lines and wrinkles, minimize pores, stretch marks, scars and much more.
This is an awesome treatment! In the microneedling treatment we use a handheld pen (I use MD Needle Pen ) to create controlled depth micro-injuries to the skin. The micro-channels created by the procedure allow for optimal absorption of topical products, enhancing their effects in the skin’s deeper layers. This stimulates the skin’s natural repair process to produce collagen and elastin reducing the appearance of so many skin concerns.So what happens in the room when we are doing this treatment?
We will start by taking pictures of the area that we will be treating. Numbing cream will then be applied to the area for around 15-20 minutes to reduce discomfort. Microneedling does NOT and should not hurt when you get it done. The numbing will be washed off prior to the procedure.
If we are doing PRP or PRF we will simultaneously draw 1-2 vials of blood that will be spun in a centrifuge to separate the Platelet Rich Plasma (PRP) or the Platelet Rich Fibrin (PRF).
When the PRP/PRF is finished in the centrifuge and the plasma is separated from the red blood cells, we will draw up the plasma and use that in the treatment as a gliding agent. Your practitioner will apply the PRP/PRF topically to the skin and then the micro needling pen will be glided over the first area of your face several times.
We will remove the numbing and then start to needle. We start on the most sensitive area first and that is the forehead – as it is bony and have thinner skin then the lower part of the face. We glide over that area with 4 or more passes so the area a covered thoroughly, before moving to the next area. The micro channels provide direct a pathway to the deeper layers of the skin for optimal rejuvenation. We continue this procedure until the whole area is treated.
Microneedling is an ideal treatment for smoothing out lines, wrinkles, skin texture, scars and so many other skin issues, and also just increasing plumpness and overall rejuvenation of the skin.
RETINOIC ACID – THE POWER OF VITAMIN A
Retinoic acid, also known as Tretinoin, is a derivative of vitamin A and belongs to a class of compounds known as retinoids. It’s a potent ingredient widely studied for its numerous benefits in skincare, including anti-aging, acne treatment, and skin brightening. I talk about Tretinoin or Retin-A a lot and you can also use that as a peel or solution, not only as a topical cream. Retinoic Acid has many names and they all have different strengths; Retinoic Acid, Retinol, Retinaldehyde, Retin-A, and Tretinoin – are all Vitamin A.

It can also help improve skin texture, tone, and firmness, resulting in a more youthful-looking complexion. Retinoic acid is also a skin brightener and can help improve skin discoloration by increasing cell turnover and promoting the shedding of dead skin cells, which can lead to a more even skin tone and texture.
- Anti-aging: Retinoic acid is highly effective for reducing the appearance of fine lines, wrinkles, and age spots by stimulating collagen production and accelerating skin cell turnover. It can also help improve skin texture, tone, and firmness, resulting in a more youthful-looking complexion.
- Acne treatment: Retinoic acid is also effective for treating acne by reducing oil production, unclogging pores, and preventing the formation of new acne lesions. It can also help improve the overall appearance of acne-prone skin by reducing inflammation and hyperpigmentation.
- Skin brightening: Retinoic acid can help improve skin discoloration by increasing cell turnover and promoting the shedding of dead skin cells, which can lead to a more even skin tone and texture.
- Side effects: Retinoic acid can cause skin irritation, dryness, and sensitivity, especially in the early stages of use. It’s important to start with a low concentration and gradually increase as tolerated. Retinoic acid can also increase sun sensitivity, so it’s important to wear sunscreen and avoid prolonged sun exposure while using it.
- Prescription-only: Retinoic acid is available only by prescription from a dermatologist or healthcare provider, and it’s important to use it as directed to avoid potential side effects and maximize its benefits.

HOW LONG DOES WRINKLE RELAXERS LAST?
How long will this wrinkle relaxer last asks a lot of patients. I would like to know that as well if I was new to getting them. Well, if you get full correction, meaning the correct number of units for that specific area that you as the practitioner will recommend them to get, the effects from a single treatment of Jeuveau are similar to Botox which will normally last 3 – 4 months.
I always inform the patient that doesn’t mean that it will disappear on the day of the 3rd or 4th month, no that means that it will gradually wear off during that time, and you might have full movement again when those months have passed. The model in this video got a very light dose, so she will have movement and it will wear off much faster than if she got a full correction.
What is full correction then? Do you explain this to your patients so they understand? The recommended units for the upper face meaning 3 areas only which are glabellar (11’s between the brows), forehead, and crow’s feet are 64 units. Some need even more than that, me for example. I have very strong muscles and I get more units than that, last time I got 76 units if I remember correctly. I do a lot around my eyes, so it adds up. So when I hear someone saying they are on a budget and asking the practitioner to only use 20-30 units “to sprinkle a little bit all over” that drives me cray cray. That is something we do not do and I hope you do not do that either. The results will not be great and the patient will then complain saying that the product didn’t last that long.

RHA 4 + Redensity Dermal Filler Part 1
Here we are more of the Resilient Hyaluronic Acid (RHA) filler, but here we are filling the nasolabial folds (NFLs). That is the line that runs from the nostril to the corner of the mouth and unfortunately gets deeper with age. The fat pads on our face get less plump the more mature we get, and the only fat pad that kind of does not lose its volume is the one that is between NLF and the cheek, so the fold there gets deeper as the fat pad on the other side of the fold move lower (gravity) and in turn the fold gets even more prominent. Here we are doing RHA 4 to first give the fold a lift and in the second video we will “retouch” with Redensity.
It is not just one filler that does it all, they all have their place and area depending on how the patient’s face is structured and where the volume loss is. They all have different textures and thicknesses, the ones that are a little thicker will go deeper and can build more structure, and the fillers that are thinner can go more superficial to correct the very thin lines and wrinkles. We usually have to inject a combination of both thick and thin for the best results. We are layering them, just like a cake. Do you layer the fillers you work with as well?
RHA 4 gives a natural volume for deeper dynamic wrinkles and folds. RHA 3 and 2 result in elegant and refined smoothing for those seeking a soft, natural look. Redensity is a weightless filler that smooths delicate lipstick lines, this is the retouch filler.
MORPHEUS FOR STRETCH MARKS – ARMS
We just finished a series of 3 Morpheus treatments on a patient that was treating her upper arms (only the medial and some on the posterior upper arm) and minimal on the lateral chest for stretch marks.
This beautiful patient had very low self-esteem and it was definitely more of a therapy session at her first treatment visit. She was incredibly nervous and as she did not like her body to start, she did not know what to expect from the treatment. At her first treatment visit my nurse went in and took pictures and applied numbing. She gave me the load down, but as she had to take care of another patient in between this patient numbing until we would start her treatment I popped into her room for a chat.
As these situations can happen, it is extremely important to have a positive attitude when you work and when you are around any patients and/or clients. If you have shit going on in your personal life, make sure you leave that at the door walking into your workplace. The energy is contagious and everyone will feel it, whether you like it or not. Believe me, sometimes life gets the best of me and I have a hard time keeping my emotions at ease too – some of my staff have experienced that. What to do in those situations – you apologize and explain why it happened but do not come up with excuses as there should be none. You need to take responsibility for your own actions and put your big girl panties/pant on! I have a few other videos about that as well if you would like to check those out.
Ok, so back to this pateint. I walked in with a smile and I saw that she was tense and nervous. I sat down next to her, so we have eyes at the same height, you do not want to tower over someone – or stand behind them – when you are communicating and asking how they are doing and feeling etc. After a few minutes, I saw that she relaxed, and she even told me so herself. (I have another post about how to go about this as well) I asked her questions about why she had low self-esteem and also gave some realizations that she had not thought about. Before she knew it herself she was smiling and said “I can not believe that you just made me feel so calm after being so nervous about this” – that made me smile as well :).
So after being with her for 45 minutes, it was time to switch rooms and start with the treatment. She did incredibly well and as we do a very aggressive Morpheus treatment with double or triple the passes from a “normal” treatment we are always two practitioners in the room to hold the chiller, massaging the patient and keeping them distracted – which was such a relief for her.
This patient was Fitzpatrick 5, so we did not use 1mm on her. For the first treatment, she wanted to do some fat reduction around her axilla. but it was not needed during the 2nd and 3rd treatment times.
The last time we did mesocaine or mesoram as well, and the discomfort or spiciness on the “Spicy Scale” from 1 being “nothing” and 10 being “kill me I never want to do this again” she was between 0-2, which was amazing!
She is now done with her series for the arms – AND she now wants to come back to do other areas. I have her treatment settings below. We used the 24 needle cartridge tip.

TRAINING STAFF & CO WORKERS

Pretty good quotes, don’t you agree? So obviously you know or have figured out by now that I like learning, but do you learn and train your staff or co-workers when they are in need?
I think it is incredibly important to train your staff so you know that they will do the best treatment possible (if it is in aesthetics). If you are in another field it is equally important. If you have someone on your team that says; “I feel like I know all of this already, I have nothing more to learn here”, then it might be time to either have a talk with that person to really see what is going on or if you are in a workplace that training isn’t important, maybe time to find something else and a place that you will develop and learn more? You tell me.
Have you ever been in a similar situation? When someone tells me that they know everything already, I discretely shake my head and in my mind thinking “this person is definitely not for me”. How can someone say they know everything – and trust me this has happened several times in the past where I have been working. I have also had staff members that have expressed that to me, but they just didn’t have the drive to want to learn. So how do you train your staff?
